Editor’s note: The below recap contains spoilers for The Pitt Season 2 Episode 4.

It’s been a rather slow first three episodes for this season of The Pitt so far, at least in terms of the cases Dr. Michael “Robby” Robinavitch (Noah Wyle) and company are dealing with. Instead, it’s the simmering drama under the surface that’s been the focus, whether that be Robby’s distrust of Dr. Baran Al-Hashimi (Sepideh Moafi), or his not taking care of himself mentally or physically on his last day before sabbatical. However, as Episode 3 ends with the reveal of another hospital closing down for a mysterious reason, the ER is about to get a lot busier. By the time the end credits roll, we might know why, and it’s far from good.

Santos Isn’t a Fan of Al-Hashimi in ‘The Pitt’ Season 2 Episode 4

Episode 4 begins with the staff taking bets about what could be happening at Westbridge Hospital that’s resulting in their patients being moved to the Pitt. Dr. Robby wagers that the Code Black is for flooding, but Dr. Al-Hashimi bets with a smile that it’ll be worse. In the waiting area, the tased college student’s sister is brought in to see him, and informed that he needs a CT to find out what’s wrong. Meanwhile, Donnie Donahue (Brandon Mendez Homer) is treating the Deaf patient, as Dr. Frank Langdon (Patrick Ball) helps a woman with a cough who needs further evaluation.

A conversation between Victoria Javadi (Shabana Azeez), Dr. Trinity Santos (Isa Briones), and Dr. Dennis Whitaker (Gerran Howell) begins with the latter talking about how being roommates is going, before turning into how Dennis is “playing” house with a farmer’s widow. He insists the woman is only a friend whom he’s helping out. Al-Hashimi encourages Santos to get her charts done faster for timely billing. Santos isn’t happy but holds her tongue when her soon-to-be boss mentions that she might have to repeat her R2 year if she doesn’t improve.

The staff is standing outside, waiting for ambulances from Westbridge to show up, when Santos comes right out and asks if anyone likes Al-Hashimi. That only gets a laugh in response from Robby. Patients start arriving, one ambulance after another, when all Robby wants is one minute for a breather. The worst is a barely conscious man who fell while doing parkour. His partner nonchalantly films the whole thing for TikTok before being escorted out.

Whitaker goes to Robby for help with a patient at the same time as the wife of the diabetic man comes in to see her spouse. She talks to Dr. Samira Mohan (Supriya Ganesh) about how they can’t afford treatment because they don’t have health insurance, so the doctor goes to get their case manager, Noelle Hastings (Meta Golding), to help. Mohan’s confused about how her patient and his wife have jobs yet no health insurance, but it’s because they work multiple part-time jobs that don’t offer insurance, and they make too much for Medicaid. She brings up the ACA as a suggestion. Their daughter overhears the conversations and is shocked by the news. Meanwhile, her father’s symptoms are only getting worse because he can only afford to use half of his medicine.

‘The Pitt’ Season 2 Episode 4 Reveals What Happened to Collins

Langdon’s back to helping the woman with the superglue in her eye. Mineral oil didn’t work, so it’s time to trim her eyelashes. As he does that, Whitaker is with a patient who has a heart attack right in front of him. Robby and others come in to help, quickly getting him back. While this is going on, Dr. Mel King (Taylor Dearden) talks to Santos about how nice it was to have been flirted with earlier, even if the patient had turned out to be a criminal. Together, they treat the woman with the cough, but Santos is distracted, not speaking to the patient at all, and antsy to leave.

Joy Kwon (Irene Choi) and James Ogilvie (Lucas Iverson) are being observed by Robby, and Joy accidentally cuts her hand while treating a man. Now she’s a patient who will have to get a blood test. Al-Hashimi wonders if they should bring Langdon in to help, but Robby deflects with an answer about easing him back in on his first day. She then teases him about his bet, and with a smile, tells the doc she’ll buy him a drink with her winnings. Robby watches her walk away, surprised that she said that, but in a good way?

Javadi talks to the college kid’s sister. He’s still unconscious, but all of his tests are coming back normal. They can’t figure out what’s wrong yet. A less serious case has Dr. Cassie McKay (Fiona Dourif) talking to the old man who’s been flirting with her. He’s been diagnosed with a fractured tailbone. He hopes it doesn’t slow him down because he’s got a date every single night this week. He wants to heal quicker, so Cassie suggests a painful procedure that can push the bone fragments back in place. When she turns around, the young, muscular man with the injured foot is staring and smiling at her. Cassie starts to walk away, then turns around and asks him out. Later, she treats the man with the broken tailbone, which means putting her finger in a place he’s not quite comfortable with. However, with a quick shove, she makes everything better.

Whitaker and Ogilvie go to Louie to treat his bad tooth. They’ll give him a temporary fix, but he needs to get it pulled. He asks about Dr. Collins (Tracy Ifeachor), and Whitaker tells him that she left, having finished her residency, and how she’s an attending in Portland, where she’s adopting a baby and wants to be closer to her family. Robby overhears this and looks heartbroken.

Robby and Langdon Are Being Forced Back Together in ‘The Pitt’ Season 2 Episode 4

Cassie talks to the parkour guy’s partner. He has a bad concussion, but he’ll live. Or will he? He’s now bleeding badly from the back. There’s a loose piece of glass that Ogilvie starts to pull out, but it’s pinched up against a blood vessel. He’s just made it worse and has his pride taken down a peg or two, but they’re ultimately able to get the shard out and stop the bleeding.

Santos is visibly down as she struggles to catch up on her charting, while Mel wonders out loud if the woman they’re treating has an eating disorder. The patient looks nervous when asked if she’s throwing up a lot. Santos checks her teeth and finds a lack of enamel. Her pneumonia is caused by an eating disorder, but she doesn’t want to talk about it. Later, the patient admits the truth to Mel, and the two set her up with an outpatient eating disorder program. Mel offers an open ear to Santos as well, because she knows being an R2 is intense, and offers a suggestion: dictation to finish her charts faster.

Javadi presents the tased student’s case to a psychiatrist, but he can’t talk to the guy when he’s still sedated. In the meantime, Robby talks to him about a motorcycle patient who just died, even though it won’t stop him from taking his own trip. The psychiatrist has the name of a therapist for Robby to talk to, but Robby only hopes they’re better than the last two. Whitaker gets in on the betting about what happened at Westbridge, while Langdon is shocked that the woman with the superglued eye still isn’t able to open it. The patient asks for “Dr. J,” one of the best doctors in Pittsburgh, and pulls out her phone to show Langdon a TikTok video of Javadi.

Langdon checks on Louie, who’s feeling much better, before Al-Hashimi goes against Robby’s wishes and tells Langdon to work with them. Robby tells Dana Evans (Katherine LaNasa) he was hoping to be on his sabbatical when Langdon came back, but Dana thinks it’s a sign that they’re supposed to talk. Langdon then goes back to check on a woman with a leg injury, and her infection is getting worse. Donnie Donahue (Brandon Mendez Homer) wonders aloud if it could be MRSA, a superbug infection. Langdon looks very concerned. “Or worse,” he adds.
